Subject Radiosynthesis of [68Ga]Ga-PSMA-11 with customized synthetic module for routine clinical use

Abstract [68Ga]Ga-PSMA-11 ([68Ga]Ga-PSMA-HBED-CC) has been widely employed as efficient PET imaging agent for early diagnosis of prostate cancer. For the routine supply of prostate-specific membrane antigen (PSMA) radiotracer in the clinics, we aimed to develop a reliable method to produce [68Ga]Ga-PSMA-11 using in-house built radiolabeling platform. Herein, we report the automatic radiosynthesis of [68Ga]Ga-PSMA-11 for routine clinical application. For the optimal 68Ga chelation, the first eluate (1.5 mL, 0.6 M HCl) from SnO2-based generator (iThembaLABS) was discarded. [68Ga]Eluate (2.5 mL, 1332-1369 MBq) from generator was introduced directly to the vial containing HEPES powder (500 mg) and stirred (pH 3-3.5) while heating. When HEPES was completely dissolved at 95 oC, PSMA-11 precursor (10 μg) in H2O (0.2 mL) was added for complete 68Ga chelation at 95 oC for 10 min. Final product was purified using SPE cartridge (Strata X C18, Phenomenex) and washed with H2O (10 mL) to remove residual HEPES. [68Ga]Ga-PSMA-11 was isolated with EtOH (0.7 mL) and formulated with 0.9 % saline (9 mL). [68Ga]Ga-PSMA-11 was produced in 95-98 % RCYs (925-962 MBq, n = 10) with radiochemical purity above 98%. Overall preparation time was ca. 30 min. The purity of [68Ga]Ga-PSMA-11 was verified by iTLC (eluent: 50% MeOH/NH4OAc, v/v). The residual HEPES content was determined by HPLC. This house-built automated module was proved to be efficient and economical protocol compared to commercially available alternatives.
